To download the .pdf file directly to your computer or to a disk, right click on the link, then select "save link as" or "save target as" from the dropdown options. Make a note of the location on your drive and the name of the file being saved so you can locate it later. 
Please Note:  The free Acrobat Reader does not allow you to save a completed, or a partially completed form on your computer. We strongly recommend that you print two copies of the filled-out form.
 

  1. Letter of recommendation from Clerk of Monthly Meeting
    or Overseers confirming membership.

  2. Letter of recommendation from the Principal of the school
    in which candidate is teaching.

  3. If candidate is not teaching, the letter must be written by an educator
    well acquainted with the candidate and the candidate’s circumstances.
